A. documents provided by the insurance company
1. Application of claim for compensation
2. Trust deed of clients
3. Power of attorney of Insurance claims payments transfer
 (The above should be signed by proposer)

B. documents provided by the proposer
1. ID card, registered permanent residence book (teenager) or birth certificate, copy of the bank book or bank card.
2. For the foreign students, the school should provide a testimonial if the guardian can’t prove the relationship between insurant and himself / herself.
3. Case history book, invoice for medical service, bill for medicine charge, the summary for leaving hospital.
4. The proof of insurance with school chop on it.
    a. disabled: to provide case history book, certificate of authenticity for the degree of disability, photos of injured part and a positive image of the insured.
    b. death benefits: provide the insured medical certificate of the death(forensic autopsy report), the cremation permit, the household write-off issued by the Public Security Bureau, the household registration or ID cards of the beneficiaries.

Note:
1. all the medical original invoice and medical expense details must be the formal medical invoice issued by the hospital's above the second-class hospital.
2. The name of the medical records and other information provided by students must be consistent with the passport or identity card.
